当院を初めて受診される場合、最後の診察から 6 ヵ月以上経過した場合は「問診票」をご記入いただいています。
事前に問診票をダウンロード・印刷していただくことでスムーズにご案内ができます。
また、ご自宅にて落ち着いてゆっくりとご記入いただくことができます。
ご記入いただいた問診票は、診療当日にお持ちください。
※当日、お越しいただいてからのご記入でも大丈夫です。
